Administrative - Application Instructions
This application has many sections to be completed and may take quite some time to fill out. Please make sure you have enough time to complete it prior to beginning the process.
Please have the following information available to you prior to filling out the application.
- Social security number
- Emergency contact
- Certificate number (if applicable)
- Certificate type, description, date issued
- Books/articles you have authored
- Additional training (conferences, in-services or seminars)
- Personal references (name, occupation, address and phone) – not former employers or relatives
- Military service record
- Past and present employment (name, address, type of business, phone number, supervisor, salary, dates of employment, reason for leaving)
- Education – high school, university, (name, address, graduation date, type of degree, overall GPA, major/minor)
- Student teaching (district name, city, state, supervisor, phone number, extracurricular activities)
Also, applicants now have the ability to send attachments immediately after submitting the application. Please note that you will not be able to send attachments once you have closed out of the application system.
